Stay Safe Homes Application

1. To be eligible to receive program assistance from Stay Safe Homes, the following criteria must be met for the Families in Recovery Placement

Must be drug free prior to entering .

• Must have 1-2 children

• Must have insurance

• Have an ID or able to obtain prior to

• NO evictions

• NO felonies

• NO drug convictions

• Must participate and attend program

• Must be enrolled or able to enroll in a AHCCCS Medicaid and SNAP benefit program

2. To be eligible to receive program assistance from Stay Safe Homes, the following criteria must be met for Sober Living Placement

• Must Stay Sober

• Must have your ID/DL

• Must be able to follow Sober Living Home requirements

• Must be employed or actively seeking employment

• Must be able to pay weekly fees until scholarship begins

Have not received sober living assistance within the last year.

Only applications submitted through this website are considered.

3. To be eligible to receive program assistance from Stay Safe Homes, the following criteria must be met for Home Sharing Placement

Must be able to stay sober and committed to your recovery
Must be enrolled or able to enroll in a AHCCCS Medicaid and SNAP benefit program
Must participate and attend program curriculum

4. To be eligible to receive program assistance from Stay Safe Homes, the following criteria must be met for the Recovery Housing Assistance
